You can check this with our practical checklist:<\/p>\n\n\n\n<ul class=\"wp-block-list is-style-kolling-arrows\">\n<li>Please fill out our <a href=\"http:\/\/gonelly.de\/kollingza\" target=\"_blank\" rel=\"nofollow noopener\">medical history form<\/a> before your first appointment. Your personal data is transmitted to us securely and in encrypted form.<\/li>\n<li>Electronic health card (for people with statutory health insurance).<\/li>\n<li>X-ray record card (if necessary).
