Checklist: before the appointment

Have you brought all of the important documents for your visit? You can check this with our practical checklist:

• Please fill out our medical history form before your first appointment. Your personal data is transmitted to us securely and in encrypted form.
• Electronic health card (for people with statutory health insurance).
• X-ray record card (if necessary). If you don’t have one, we’ll be happy to issue one for you.
• Allergy record card if necessary.
• If you have to take medication regularly: a medication list.
